Hazel Lucille Moore, 97, passed away on November 4, 2016 in Mesa, Arizona. She was born on December 22, 1918 in Clark County, Ohio.
She is survived by her children, Sondra Denson and Eugene Moore, M.D.; and her grandchildren, Reagan Denson and Connor Denson.
The family will be holding private services.
