Scott Jay Fielders, 47, passed away on July 15, 2016 in Chandler, Arizona.  He was born on December 11, 1968 in Laconia, New Hampshire.

He is survived by his wife, Jennifer (Dembiec); his children, Therron and Jordan; and two grandchildren.
